Economic Stimulus Package ?

Hi my question is i read some info on this and according to some web sites they go by social security # and or your last filed returns direct deposit. I havent gotten around to getting a change of address. I actually moved from Omaha NE to Council bluffs IA. Does this matter if there going by social security number it probably should show that i have been working in nebraska but live in cb as i am getting both ia and ne taxes taken out. If i dont get a form say because of this information is it possable i request a form?
